Use this form to request the permanent removal of your personal account data from the CDJW mobile application and website. Warning: This action is irreversible. Upon processing, your membership profile, login credentials, and personal contact information will be purged from our active systems.
 

Identity Verification Requirements: To protect your security and ensure we are modifying the correct record, you must provide the following information exactly as it appears in our membership database:

• The Email Address associated with your account

• The Phone Number on file

• The Physical Address used for your membership registration
  
  Please allow up to 30 days for our team to verify your identity and complete your request.
  Important Exclusion Regarding Contributed Media: This request does not include the removal of photos, videos, or media you have previously submitted or shared for use on the CDJW website, social media, or community galleries. As a 501(c)(3) organization, CDJW Inc. retains the right to continue displaying contributed community content and historical media even after a personal account has been deactivated. Additionally, certain transactional records (such as membership dues or donation history) will be retained as required by law for tax and audit purposes.
